How to Arrange Home Care in Somerset

You do not need a finished care plan before asking for help. Start with what is becoming difficult, what matters to the person and where support is needed.

1. Prepare a short picture of everyday life

List the tasks that feel difficult, the times when help would be useful and the routines the person wants to keep. Include any existing family support, mobility equipment or professional involvement you would like discussed during assessment.

Keep the person receiving care involved in the conversation wherever possible. Ask whom they would like to include when planning support.

2. Explore an assessment

Somerset Council offers a care and support assessment to understand an adult’s needs. A council assessment and a provider’s assessment serve different purposes: the provider must also establish whether it can deliver the proposed support.

If you live in Bath, Midsomer Norton or Radstock, use Bath and North East Somerset Council’s assessment information. Confirm which council covers your home address.

3. Discuss the service and location

Compare visiting care, live-in care and, where relevant, complex care. Ask what each arrangement would mean for the person’s actual day.

Selwood Care welcomes enquiries from Shepton Mallet, Frome, Bruton, Wells, Glastonbury, Wincanton, Bath, Midsomer Norton, Radstock and Somerton. See our areas we cover and ask about availability at your postcode.

4. Confirm the details before care begins

  • The tasks, visit schedule and responsibilities in the care plan.
  • The proposed start date and how it will be confirmed.
  • The full quote, payment terms and any additional charges.
  • Who to contact with a question or concern.
  • How changes to the arrangement will be agreed.
